Remarks: On a paper slip attached to the inner margin of 1, 1 recto, in the same hand as in V.4.10, is the following: Nicolaus [an obliteration] seu | Prepositus vixit circiter A.C. 1198 | Nicolaus *Hostersham an English | man and notes Physician wrote | says Bale de Scrip: Brittan: an Antidotary beginning thus ego Nicolaus rogatus a quibusdam etc. | "so called Dr. Fuller supposes from | Horsham in the county of Sussex | contra dolorem Renum l. 1. Paper, 10 3/4 x 7 3/4, ff. 64, written (?in France) in double cols. of 32 lines each about 8 x 2 3/8, margined, but not ruled, with brown crayon, no signatures or catchwords visible, foliation, modern, in pencil (1-60), illuminated initials (blue with red ornament and gestures), blue and red initials, initials of sentences touched with vermilion, few marginalia, two folios mutilated (28 [not numbered] 29 [30]), dirty, edges torn and repaired, cropped, the paper has the same water-mark as V.4.10, fol. sec. modum auellana. Cent. XV.
Binding: Millboards, the same binding exactly as V.4.10, title (gilt) on dark green morocco shield: NICOL | HOSTRES | ANTIDOT | [shield lost] M.S. [impress only]. Cent. XVIII.
