On 1, 3 verso are four lines in a XVI. Cent. hand of a prescription in Latin.
On 1, 4 verso are noted nine lists of drugs which agree in their properties, beginning: gomme quae possent temperari in aceto. In the left hand bottom corner is a pen and ink drawing of a physician in sleeved robe, cap, cowl and cape, holding a wide-mouthed bottle in his right, and in his left a scroll inscribed: O mater dei memento mei.
On a slip of paper, pasted along the inner edge of this page, is written in a XVII.-XVIII. Cent. hand: Johannes Platearius Salernitanus. | claruit A.C. 1300 de Simplici | Medicina Liber Inscriptus Circa Instans. Quo Simplicia Medica= | =menta usitatiora Alphabetici serie | describuntur. - scripsit similiter | Expositiones et commentationes ad | Nicolai Antidotarium venetiis 1498 | Practica Brevis morborum curandorum | etiam Febrium una cum Libro de | Simplici Medicina Lugduni, 1525.
On 3, 8 verso is a note (in a contemporary hand): quere residuum in folio cxxij.
f. 21 recto has two medical receipts (XVI. Cent.) on what was then left blank: lines 39-44 on f. 20 verso are repeated on f. 21 verso lines 1-6.
De Renzi, Coll. Salern. i. 228-34, is inclined to assign this work to Joannes Platearius, a cousin (probably) of Matthew Platearius the Younger.

Paper, 10 5/8 x 7 5/8, ff. 74, originally (probably) ff. 78, written (?in France) in double cols. (except first two fols. in three, and ff. 50-53 single cols.) of about 45 lines, each 8 x 2 3/8, margined (in No. 2) with brown crayon, not lined, no catchwords or signatures visible (probably cropped), foliation, modern, in pencil (1-70), first folio and last two mounted, one illuminated initial with gestures, red and blue initials, few rubrics, rubricated paragraph marks, initials of sentences touched with vermilion, no marginalia, much cropped, had originally strips of vellum in centres of quires, in fairly good condition, fol. sec. opilationem splenis. Early Cent. XV.
Binding: Millboards, covered crimson (faded to brown) grained Russia, gilt-tooled edges, sides and panelled back, four stamps, in panels, of double-headed crested eagle (?) (same as in V.4.11), saffron edges, title (gilt) on dark green morocco shield: PLATEAR | DE | SIMPL. | MEDIC. | [shield gone] M.S. [impress only]. Cent. XVIII.
