The remaining pages are occupied with two indexes, the first beginning (29, 4 recto lines 1-5): "The table or kalender of this booke | wherbie thou may turne bi the | letters and figures to any thing | which thi hart desirith in this | booke, and that with spede," and ending (30, 1 verso line 3): "ynck - 174," and the second beginning (30, 2 recto lines 1-6): "The table or kalender of al the notable experiments | and medicines contened in the booke intituled | the secrets of Master Alexis wherebi thou may turn | bi the letters and fygures to any thing which thi hart desyreth in that booke and that with | spede," and ending (30, 3 verso line 24): "nawcea - 295."

On 30, 6 recto in an early XVII. Cent. hand: Henrye Swinburne. See T.8.17 (Description). The verso of this last folio, formerly attached, preserves the impression of the original black leather covers and two silk tags with which it was tied.
Arderne's Surgery (No. 4 above) consists of four portions as follows:

The same hand which collated John Arderne's "Practica Chirurgiae" in T.5.14 has gone over these portions and inserted marginalia and interlinear additions and corrections from U.4.9 (described as "Dr. Mead's MS."), which see. The foliation is in great part erased and re-written.

Vellum, 7 7/8 x 6, ff. 250, originally ff. 254, well written, in several hands, in single cols. of about 31 lines, each 6 x 4, margined with plummet (or pencil), not ruled, catchwords (each page), foliation (from 2, 1. to 29, 10. = ff. 1-241), no signatures, much cropped, a few rude pen and ink drawings. Cent. XVI.
Binding: Not original, millboards, covered spattered calf, gilt panelled back, title: MEDICAL | & CHIRURG. MS. Cent. XVIII.
