The MS. originally consisted of the vellum leaves 6, 1.-10, 8. and 12, 1.-27, 7., which alone have signatures and catchwords. 22, 3. preserves the signature (kiij) which shows that the 12th quire was originally the first quire in the book. All traces of signatures have gone from 6, 1.-10, 8., which may have originally followed the 27th. The 10th and 25th, having each catchwords, were evidently not originally final quires. 27, 7. (now bound between 26, 4. and 27, 1.) has the lower third cut off, and has a XVI. Cent. note: "Of the degrees of herbes and other thinges and what a degree is." 27, 6 verso has a scribbled note in XVI. Cent. hand: "To paie for this at the terme - 19s. 6d. | William Lugger which I gaue punsted" (?signature). 28, 1 rect-31, 2 verso are ruled and lettered in red (XVI. Cent. hand) for an index with the heading: "A table of medecynes and to finde them redily." The same hand as wrote the title on 6, 1 recto: "Of the necessetye of ye knoledge of Anathomie," has inserted several references, and inserted in the margin of the text the appropriate receipts, etc. A loose sheet is inserted in the end (between 33, 7. and ii, 1.) containing in a XVII. Cent. hand (34 lines): An excellent water healing Fistulas, and all | old sores and wounds.
There are traces of pounce on the margin of 10, 7 recto.

Vellum (6, 1.-10, 8; 12, 1.-27, 7 = ff. 154, originally ff. 156+), paper (1, 2.-5, 6; 11, 1-8; 28, 1.-33, 7 = ff. 84, originally ff. 88), 10 3/4 x 8, ff. 239, originally ff. 244+, well written in an English hand, in double cols. of 30 lines, each 7 x 2, margined (brown crayon) and ruled (plummet), (vellum only), signatures (traces), catchwords (contemporary), pagination (XVII. Cent. in black and red, and modern foliation in black, all partial), gilt illuminated initials and blue initials (with red gestures), blue paragraph marks, numerous marginalia in black and red, no signatures or catchwords in paper quires, rubricated running titles (XVII. Cent.) vellum defective in places, fol. sec. for to tellen. Cents. XIV. and XVII.
Binding: Millboards, covered rough brown calf, blind-tooled, red mottled edges, title (in ink) on a slip of paper: CHAULIAC, etc. Cent. XVII.
